If you did, please complete our Juror Qualification Questionnaire within ten (10) days of receipt of this postcard. To complete the questionnaire, you can scan the QR code on the front of the postcard and it will take you to the E-Juror application or visit E-Juror at https://ejuror2.uscourts.gov/ejuror-nynd/TransportRoom?servlet=login. You will need your nine (9) digit participant number, located above your name on the front of this postcard. Paper Juror Qualification Questionnaires were mailed on March 13, 2026. If you have not yet completed your questionnaire, you may still do so online at any time or by returning the paper form. Please note that you only need to complete one version of the questionnaire – either online or paper.
