FORMS Pursuant to the Announcement from the Second Circuit Court of Appeals, effective July 15, 2008, all CJA vouchers and voucher worksheets must be typewritten. Vouchers or worksheets received handwritten will be returned to counsel. You can fill out the forms on-screen and save them before submitting to the Court for review and consideration.
Circuit Notice to All CJA Panel Members regarding billing records CJA Panels of the District and Circuit Courts of the Second Circuit are required to maintain contemporaneous billing records for all work performed on CJA cases to which they have been assigned, including work performed by associates, partners, support staff, as well as expense records. CJA counsel are required to maintain these records for a period of no less than three years for audit purposes. |

Unofficial Dual Language Forms Guide to Judiciary Policy, Vol 22, Ch 1, § 150.15 Unofficial Dual Language Forms states: The Judicial Conference's Committee on Court Administration and Case Management has endorsed the posting of certain dual language forms that may be of assistance to persons of limited English proficiency. Each form contains the complete English text of the national judiciary form in addition to a Spanish language translation below each line of English text. These forms are not nationally supported or officially approved, but are provided as examples only as a matter of convenience to assist court interpreters and litigants with limited English proficiency as well as their attorneys. They do not replace the need for an official form in the English language. The availability of these forms does not obligate the judiciary to produce comparable forms in any other languages used by the public or defendants in court proceedings initiated by the United States. The Administrative Office of the U.S. Courts takes no responsibility for the accuracy or legality of the material contained in these forms. *Note* - Only English version forms will be accepted for filing with the Court.
